If you are a provider, agency, or individual referring someone to Foothills Recovery, please complete our referral form to begin the process. This form helps us gather important information about the individual’s background, treatment needs, and current situation. Once submitted, our staff will review the referral and follow up promptly to coordinate the next steps. Referrals may be faxed or emailed using the contact information listed on the form. We appreciate your commitment to helping individuals take the first step toward recovery.
Who Can Refer
We accept referrals from healthcare providers, social service agencies, court officials, family members, and self-referrals. If you believe someone could benefit from structured, supportive addiction recovery services, we welcome your referral.
What to Expect After Submission
Once the referral form is received, our intake team will review the information and reach out to the contact listed. We’ll discuss next steps, verify eligibility, and coordinate any additional documentation needed.
